Insurance Policy

At Premier Women’s Health, we want you to be fully informed and confident about the services and fees of our obstetrics and gynecology center.  We accept most major credit cards and insurance policies.   We also accept Care Credit.

Self-Pay

As a courtesy to our patients who are uninsured, we offer reduced rates for some of our services. We also provider very affordable obstetrical packages for our self-pay pregnant patients. This does not include hospital or laboratory services. Please call our office to discuss services and rates in detail. 

We will require that patients with self-pay balances do pay their account balances to zero prior to receiving further services by our practice. This does not apply to Self-Pay Obstetric patients.

Insurance

It is your responsibility to contact your insurance company and determine deductibles, percentages due, co-pays and non-covered services/charges.  If you have any questions, please do not hesitate to contact our billing department at (228) 206-1905, option #4.

Payment:  Co-payments, balances from prior visits, and other amounts that may be due on your account are due at the time of your visit.  If you are not prepared to pay the amount due, you may be asked to reschedule your appointment with us.

Prescription Refills:  We request a minimum of 48 hours for all prescription refills to be processed.

Returned Checks:  A $40.00 processing fee will be charged for all returned checks.

Family Medical Leave (FMLA) and/or Disability Forms

If you will need Family Medical Leave (FMLA) and/or Disability paperwork completed for your employer, there is a fee of $25.00.  This is to be paid when forms are dropped off to be completed regardless if they are to be faxed to the employer or picked up by you. You MUST fill out your portion of the FMLA paperwork prior to dropping it off at Premier Women’s Health. We CAN NOT fill out our portion unless yours is already filled out. You MUST also allow us a minimum of one week for completion of these forms unless an emergency situation prompts the necessity of them.
